If I have Surfshark already installed and running, does the browser extension just duplicate what the program does or do I need that as well? Also the reviews all say the extension doesn’t work well for Firefox. Does anyone have any experience with that?

The browser extension is just a proxy anyways and not “really” the same as the actual VPN client itself.
The difference being, the browser extension encrypts all the traffic done in that session (in that browser) whereas the VPN client is all Internet traffic on the device.
i.e. I have my PC with Chrome, Microsoft Teams, Spotify, Mail App all open. The browser extension for chrome hides all that data, but my ISP etc can “see” the traffic from Spotify and the others.
When I click on view devices, it lists my laptop, my phone, and my Chrome browser. Do you have any idea why it lists my Chrome browser?

Yes, its not necessarily a “device” list more a "login " list.
Like if you install now fire fox , or brave or something and log there in it would list this again as device.
Dont worry about that surfshark knows which devices are active and its mostly a thing for your security so you could de authorize devices and stuff i guess.

Dont use a VPN with google , or use Bing or any alternative search engine.
Sadly Google is extremely vpn hostile and any vpn out there got the same issue with captchas and google.
